Appears to be private purchase cap, typically for officers, cant comment on the badge and cant see whats on the buttons

Buttons look like OSD General Service, is there a name written on the label in the cap?

Yes, the buttons are General service. And no name on the label

Yes, it is an original cap and tailor made as for officers as expected. The cap badge has lugs (normally blades), but that could be possible too. Can't tell how long it has been on the cap.

This looks like a nice private purchase. An OSD collar badge has been used here, but I have seen numerous examples of this being done with this trype of head dress.
