Default 1st Nottinghamshire RVC cross belt plate

I have just acquired this well used and abused cross belt plate the 1st Nott.R.V.C..

The screw posts are no longer present and it looks like a QVC is missing from the top.

Does anyone please have a picture of a complete example they could post to see what the plate should look like ?
