Hi Rob, The discharge would be the doc I would follow. The sketch of the service that you gave supports the issue of the 39-45 Star. (It is unusual to have the F&G without having earned the 39-45 Star.) Plus you have the ribbon.
The statements of service were compilations of his service records, and it is likely an error was made at that time. You can request his medal entitlement from LAC, but it will take some time as they have experienced serious cutbacks.
