Looks Italian, possibly medical officer in Navy.

This is a cap badge of the Milizia Portuaria (an armed branch of the Italian Fascist Party which was in charge of guarding harbours and maritime installations such as lighthouses or other).
Some examples here; the cross of Savoy (nothing to do with Red Cross) indicates someone not attached to a particular Legione (a particular regional unit).
