Hi Rob, The discharge would be the doc I would follow. The sketch of the service that you gave supports the issue of the 39-45 Star. (It is unusual to have the F&G without having earned the 39-45 Star.) Plus you have the ribbon.
The statements of service were compilations of his service records, and it is likely an error was made at that time. You can request his medal entitlement from LAC, but it will take some time as they have experienced serious cutbacks.

The War Medal was issued after the war which may explain why it is not listed. The inclusion of the 39-45 Star and its subsequent exclusion may well be a clerical error and you should write to Veteran's Affairs.
The 39-45 Star was initially the 39-43 Star and ribbons were issued to qualified soldiers during the war. This may explain the ribbon. He may have had another which was the CVSM as that was also approved during the war, in ribbon form only. Cheers, Clive
