WW1 Medal Record Card - What Does It Mean?
I am looking to try and decipher the attached medal record card, but I have to confess that I know little about what the coding /notes actually mean. So far I have this:
Charles Frederick Wake Lieutenant in South Staffordshire (Regiment?) Machine Gun Corps Victory Medal War Medal Badge No. 383946 (Is this a war badge?) Theatre of War first served is France The rest is a mystery to me (including the notes on the back) Any help in deciphering the rest would be greatly appreciated.

Yes its his wound badge no. Wounded whilst a Lieut in the Machine Gun Corps.

The reverse shows he filled in a form ef9 and applied for his medals. Form ef9 was a list of his service and had to be completed before he was ussued his medals.

At the very top looks to be a penciled notation dated pre WWI which usually means he served before the war, possibly with a different regiment, I can't quite read it. This often means there might be another card for him or if his service records survive you can see when and where he served. In this case with a second read it seems to read BW & WM IC 100 15/11/?? seemingly referring to another MIC.
One thing, you were correct it is a silver war badge (SWB) not a wound badge as Andy commented, they were given for a number of different reasons and most were not to do with wounds. to the right of the two meadls mentioned and which Andy said were awarded for his service with the MGC as a LT (shown by the X next to the medals and his rank/unit. The bracket show they were issued together, the IV is for issue voucher which shows he signed for them by return of such, the WD is for war department clerk and the date and NW is uncertainly thought to be for New armies. You can get his SWB roll from ancestry and that will give you an annotation for why he was discharged mid war.
