Its not specifically Canadian or Nursing.

Its a Victorian General Pattern Officers Levee wear waist belt clasp for Volunteers as its Silver with a Gilt Royal Crest.

Worn by officers in many units of Infantry and Arms and Services.

I think Simon is spot on. Here is a KC version c1901-02, on the dress sword belt.
