Victorian Pouch Badge?
I'm taking a guess by labelling this a pouch badge. I don't think I'm wrong in calling it Victorian. It measures 75x76mm and appears to be gold gilt, with three nut & bolt fittings.

I think it could be a NSW Medical Staff Corps (1888-1898) cross belt badge.
Grebert say it is 77mm in height which is close to what you described. Reference: Grebert's 'Military Badges of the Australian Colonies' p 139 Mick

Looks very like a Depot Training Battalion staff plate, C.1860.
