Default Sanitary service?

It was once suggested to me that this badge was Sanitary service and that it was a volunteer establishment during the great war. Any help with this one?

Is that a representation of the floor plan of a rather confusing 5 hole khazi?

ANGLE IRON;
It is an American Expeditionary Force Service Of Supply patch. With better pictures (especially of the back) we could tell if it is a period piece or a post WWI redo.
