D Force History

I have in front of me an interesting article on D Force, the Burma deception unit, which appeared in the much lamented Crown Imperial written by Lt Col E Turnbull MC who commanded D Force.

My copy does not have a date of publication and I wondered if anyone can provide its publication date. Did Crown Imperial ever produce an index?

Any help appreciated.
